Diabetes

Diabetes affects a significant proportion of Brent’s population and the HAZ programme will work with community organisations and the tPCT to tackle this through a range of innovative projects and by focusing on marginalized groups.

Brent PCT Community Diabetes Project

The aim of the project is to run education courses for those newly diagnosed and those with established diabetes will be taught by health professionals. The project will also produce a quarterly newsletter in various languages with information and advice about diabetes as well as form links with other Diabetes education programmes.

Brent Deaf People’s Group (BDPG)

The aims of the project are to provide accessible information on Type 1 and Type 2 diabetes for deaf and hard of hearing people using themed workshops around Diabetes such as healthy living, the 2 different types of diabetes and information management using health professionals.  The organisation will develop training for the deaf and hard of hearing people through British Sign Language (BSL) videos and leaflets. Other forms of visual communication will also be used such as role-play and drama.

Brent Community Physical Activities Project (BCPAP)

The aim of the project is to train community exercise and fitness instructors to work within community settings and establish new physical activities programmes around the borough focusing on young and disabled people. The project will map existing exercise classes in Brent and establish a community organisation that will continue the life of this project after the HAZ funding ends.

The aims of the project are to develop accessible material and also to raise awareness about diabetes testing for service users as well as parents, carers, residential home staff and expert patient programmes. The organisation will develop and distribute leaflets and posters in accessible formats to users.

The aim of the project is to set up Hospital Information Desk at Diabetes Clinic informing patients of services to assist them both inside and outside of hospital environment.

Elders’ Voice / New Testament

The aim of the project is to raise awareness of Type 2 diabetes amongst elders with a predisposition to diabetes and help those elders with the condition to make the lifestyle changes that are necessary to keep their diabetes under control.
